The 2027 benefits gap: What Gen Z, Millennials, Gen X and Boomers actually want from workplace health

We surveyed 6,305 full-time UK professionals.

Key findings

  • 51.1% paid out of pocket despite having PMI, and one in four spent £1,000 or more
  • Half of Gen Z say they’re likely to leave or cut their hours over their benefits
  • 86.5% of employees say better benefits would make them more likely to stay

We surveyed 6,305 UK professionals across financial services, legal, professional services and tech. Every one of them has PMI or a health benefits package through work, yet half still paid for care themselves last year.

The gap looks different at every age.

Under-45s want fertility, neurodiversity and mental health support. After 45, men’s health and menopause take over. Mental health is the one need that stays in every generation’s top three. Standard PMI excludes or only partly covers most of these. Younger employees feel it most: they’re paying out of their own pockets, losing the most working days, and they’re the most likely to leave over it.

This report breaks down what each generation needs and what the gap is costing you.

It also shows how to close it before equality action plans, including menopause support, become mandatory for employers with 250+ staff in spring 2027.
