What are GLP-1s?

GLP-1s (Glucagon-like peptide-1 receptor agonists) are a class of medications that have traditionally been prescribed to treat diabetes. In recent years, they have become a popular medication for weight-loss.

They work by targeting the areas of the brain that regulate appetite and food intake, this happens in three main ways:

1. Insulin secretion: They prompt the pancreas to release the right amount of insulin when blood sugar levels are high.

2. Slower digestion: They slow down "gastric emptying," meaning food stays in your stomach longer, making you feel full faster and for more time.

3. Appetite suppression: They signal the brain to lower hunger levels and reduce "food noise" (persistent thoughts about eating).

Common medications

While many people refer to them all as "Ozempic," there are several different types depending on the primary goal (diabetes vs. weight loss). Here are the names of, active ingredients in and primary uses for some of the most well known:

Ozempic

Active ingredient(s): Semaglutide

Primary use: Type 2 Diabetes

Wegovy

Active ingredient(s): Semaglutide

Primary use: Chronic Weight Management

Mounjaro

Active ingredient(s): Tirzepatide

Primary use: Type 2 Diabetes

Zepbound

Active ingredient(s): Tirzepatide

Primary use: Chronic Weight Management

Victoz

Active ingredient(s): Liraglutide

Primary use: Type 2 Diabetes

Note: Tirzepatide (Mounjaro/Zepbound) is technically a "dual agonist," meaning it targets GLP-1 and another hormone called GIP, which can lead to even greater weight loss.

Why has the demand risen in the UK?

A 2025 study found that 1.6 million adults in the UK used weight loss medication in a 12 month period between March 2024 and the following year. That’s nearly 10% of the population and a 100% increase on the previous period.

But why the sudden surge?

In 2024, it was revealed that 64.5% of adults in England were estimated to be overweight or living with obesity, with 26.5% living with obesity specifically, so clearly there is a need for effective weight-loss strategies to become widely available to the public.

While diet and exercise have and will continue to be regarded as the first port of call for anyone trying to lose weight, the reality is that most of us have busy schedules which don’t always allow the time for healthy cooking and regular physical activity. GLP-1s, by contrast, are quickly administered, effective and increasingly accessible, explaining the rising demand for prescriptions.

Does my PMI scheme cover GLP-1s?

Historically, weight loss was excluded from almost every UK health insurance policy because it was considered elective or lifestyle-orientated, alongside the likes of fertility treatments and neurodiversity support. However, the increase in demand for GLP-1s have forced insurers to reconsider their stance on GLP-1s.

While the 2026 market is shifting due to the clinical efficacy of GLP-1s, most standard UK policies still do not provide comprehensive coverage. Understanding the structural barriers within PMI is essential for HR leaders looking to plug these gaps in their wellbeing strategy.

So, are GLP-1s covered?

For most individual and small-business PMI policies in 2026, the answer is still generally no, and there are a number of reasons why:

The "acute vs chronic" hurdle

PMI is designed to treat acute conditions - illnesses that respond quickly to treatment and return the patient to their previous state of health. Insurance providers typically classify obesity management as a chronic condition because it often requires long-term medication and therefore exclude it from their standard coverage plans.

Outpatient prescription exclusions

Most UK private health plans do not cover the cost of "take-home" prescriptions. Even if an insurer covers the consultation with an endocrinologist, the cost of the actual GLP-1 pens—which can range from £150 to over £300 per month—usually falls on the patient.

Corporate vs individual schemes

If you have health insurance, your employees may be better placed to access GLP-1s quickly. Some "blue-chip" corporate schemes have begun opting into "weight management riders." These bespoke additions allow employees access to specialist-led weight-loss pathways that may include subsidised or fully covered GLP-1 prescriptions.

Breakdown of providers: Who is offering what in 2026?

As of 2026, providers are moving towards "Managed Access" models. Here are some of the UK’s leading health insurance providers and what they are currently offering:

Vitality: Offers a "Weight Management Pathway" with discounted access (often up to 25% off) to Wegovy for members with a BMI over 30 and specific health risks.

Bupa: Provides "Bupa Weight Management" as a separate add-on. This covers initial specialist assessments and blood work, though the drug cost is usually out-of-pocket.

AXA Health: Generally excludes weight-loss drugs but will cover the treatment of complications arising from weight, such as heart disease or specialised orthopaedic care.

WPA & Aviva: Coverage is usually restricted to high-level corporate plans. Individual policies remain largely "exclusion-heavy" for obesity meds.

There are specific scenarios where PMI might cover weight-related costs

Secondary conditions: If your weight gain is a symptom of an underlying medical condition (e.g., Polycystic Ovary Syndrome (PCOS), Cushing’s Syndrome, or a thyroid disorder), your policy will typically cover the diagnostic tests and consultations to treat that primary condition.

Bariatric surgery: Some premium policies cover gastric bands or bypasses if the patient meets strict criteria (e.g., BMI > 40, or > 35 with life-threatening comorbidities) and has failed all other interventions.

Mental health support: Many modern policies include "Digital GP" or mental health modules that offer nutritional counseling and Cognitive Behavioral Therapy (CBT) for disordered eating, even if they won't pay for the medication.

FAQs

Are GLP-1 weight loss drugs covered by Bupa or Vitality? Bupa and Vitality provide pathways for weight loss, but they rarely cover the full cost of the medication for individual policyholders. Vitality offers significant discounts on Wegovy through their rewards and clinical pathways, while Bupa often provides discounted access to clinical specialists who can prescribe the medication privately.

Why is obesity treatment excluded from private health insurance? Insurers cite the high, recurring cost of long-term medication and the classification of obesity as a chronic condition. However, as evidence grows that weight loss reduces long-term claims for heart disease and diabetes, some insurers are starting to offer "preventative" coverage.

Can employees get a private prescription through my PMI? Employees can often use PMI to see a private consultant (endocrinologist) who can write the prescription. However, they will likely have to pay the pharmacy the full market price for the medication.
